Track Student Goals
Goal setting helps your students to:
- think about what is most important to them
- motivate themselves
- build self-confidence based on successful achievement of their goals
A link to the Learner Goals form is below. It is meant to be completed in tandem with your student(s). Don’t think of the form as a checklist, but rather as a conversation tool to prompt and guide students about their goals.
- “What would you like to be able to accomplish at work (or at home or in the community or personally) that you can’t accomplish now?”
- “Which goals would you like to reach as a result of your tutoring sessions?”
If a goal that your student has is not on the list, write it on your form.
Once you have filled out the form with your student, submit your information here online. For goals not on the list, you can add them below under “Other”.

What is the question at the bottom of this form all about? This is a verification question. All answers are words (i.e. if the answer is 5, you need to spell out the word “five”).
